Support shaped around you

We offer a wide range of NDIS supports. We’ve grouped them into four areas so you can find what fits. Not sure? Just ask—we’ll help you work it out.

Understanding your plan and coordinating support

Help making sense of your NDIS plan and getting the right people working together.

  • Support Coordination

We help you understand your NDIS plan, find providers, set up services and keep things on track.

  • Specialist Support Coordination

A higher level of coordination for situations with more barriers — for example, when several services or systems are involved.

  • Psychosocial Recovery Coaching

Support for people living with psychosocial disability (disability that can arise from a mental health condition) to plan, build skills and work towards recovery goals.

  • Case Coordination

Keeping communication clear between you, your family and the services around you.

Everyday life and community

Practical support at home and out in the community, shaped around your routines.

  • Daily Personal Activities

Help with everyday personal tasks such as showering, dressing and getting ready for the day.

  • Household Tasks

Support with cleaning, laundry, meal preparation and keeping your home running.

  • Community Access

Support to get out and about — shopping, errands, activities and places that matter to you.

  • Social & Community Participation

Support to join groups, keep up hobbies, see friends and take part in community life.

  • Appointment Support

Someone to come with you to appointments and help you prepare and follow up.

  • Life Skills & Capacity Building

Learning and practising skills such as cooking, budgeting and using public transport, at your pace.

Complex support and transitions

Experienced support when life is more complicated or going through big changes.

  • Complex Disability Support

Consistent, well-planned support for people with higher or more complex support needs.

  • Mental Health & Psychosocial Support

Support that understands mental health, delivered with patience and respect.

  • Forensic & Justice-Related Disability Support

Support for people involved with the justice system, working alongside justice-sector stakeholders without judgement.

  • Community Reintegration

Support to settle back into community life after time in hospital, care or custody.

  • Hospital-to-Community Transitions

Working with hospital teams, families and services so leaving hospital is safe and well planned.

  • High-support and palliative assistance

Assistance for people with high support needs, including palliative support, where appropriate to the participant’s needs and CCM’s scope.

Accommodation and housing

Support to find and keep a home that works for you. Availability and funding depend on your plan.

  • Supported Independent Living (SIL)

SIL is help with daily tasks in a shared or individual home, so you can live as independently as possible.

  • Individualised Living Options (ILO)

An ILO is a flexible living arrangement designed around you — for example, living with a host or with drop-in support.

  • Short Term Accommodation / Respite (STA)

STA is a short stay away from home, often giving you and your usual supports a break.

  • Medium Term Accommodation (MTA)

MTA is temporary accommodation while you wait for your long-term home to be ready.

  • Accommodation & Housing Support

Help exploring housing options and connecting with housing services.

Good to know

The supports we can provide depend on your NDIS plan, your available funding, CCM’s registered scope and service availability. Accommodation listed here does not mean a place is currently vacant.

Not sure which support you need?

That’s okay. Tell us what is happening and we’ll help you find the right fit.
