About Us
People deserve better than feeling like a number in a system.
That belief started CCM, and it still sits at the centre of everything we do.
Why CCM exists
The NDIS can be life-changing. But navigating disability supports can also become overwhelming—especially when you or your family are already dealing with mental health, housing, health concerns, family pressures or big changes in life.
We wanted to create a service that combines strong professional practice with something just as important: human connection.
- A service that answers the phone.
- A team that communicates.
- People who follow through.
- Support that considers the whole person.

Our experience
Our team brings experience across disability services, Support Coordination, psychosocial recovery, mental health, complex support, accommodation pathways and forensic or justice-related disability support.
This experience matters most when your situation doesn’t fit neatly into one category. We know how to communicate across services while keeping one person at the centre of the conversation: you.
Who we work alongside
- Participants and families
- Allied health professionals
- Behaviour support practitioners
- Mental health teams
- Hospitals
- Plan managers

“A person may have complex support needs. That does not make the person difficult.”
What guides us
Our values
- Compassionate
- We care about the person, not just the support.
- Respectful
- We listen, value choice and treat everyone with dignity.
- Reliable
- We show up, communicate and do what we say.
- Accountable
- We take responsibility and always act with integrity.
- Inclusive
- Everyone deserves to feel welcomed, valued and understood.
- Together
- We work alongside participants, families and their support networks.
- Purposeful
- Everything we do should make a meaningful difference.
What we want you to feel
That you can trust us. Participants should feel heard, respected and comfortable having a say in their support. Families should feel confident that capable people are around their loved one—people who communicate and speak up when something isn’t right.
And everyone should feel they can pick up the phone and speak to a real person who knows who they are.
